A video showing a cycle ambulance operating in Karachi has triggered a wave of reactions on social media, with many users mocking the pedal-powered emergency vehicle and comparing it to advanced medical transport systems such as flying ICUs, air ambulances and drone-assisted healthcare used elsewhere.

However, as the debate continues, the resurfaced clip has also prompted many to defend the initiative, arguing that it addresses a real problem faced by Pakistan's largest city.

Much of the criticism was driven by viral claims suggesting that Karachi Mayor Barrister Murtaza Wahab had recently launched the service. Those claims fuelled sarcastic posts questioning Pakistan's pace of innovation.

One widely shared post read, "Welcome to the future, #Pakistani version. The world is building flying ICUs. Karachi's mayor is launching a cycle ambulance."

Another user wrote, "Inauguration of Cycle Ambulance Service in Karachi You tyrants don't even have motorcycles for the poor!"

A third post mocked the initiative by saying, "After 77 years, Pakistan finally unlocked a new level of innovation. Cycle Ambulance Unlocked. Next update: Bullock cart ICU. Donkey Metro. Camel Cart Bullet Train."

The cycle ambulance isn't new

Despite the fresh round of online criticism, the cycle ambulance programme was not launched recently.

Local reports show that the initiative began in May 2025, when the Sindh Integrated Emergency and Health Services (SIEHS-1122), commonly known as Rescue 1122, rolled out the bicycles as part of a pilot project aimed at improving emergency response times in Karachi.

The programme was designed to tackle one of the city's biggest healthcare challenges- reaching patients quickly in densely populated neighbourhoods where heavy traffic congestion and narrow streets often delay conventional ambulances.

Why bicycles instead of ambulances?

Unlike a standard ambulance, the cycle ambulance is intended to provide rapid first response rather than patient transport.

Each specially equipped mountain bike carries essential medical equipment, including first-aid supplies, oxygen masks, nebulisers and blood glucose testing kits. Trained responders use the bicycles to reach patients faster, provide immediate medical assistance and stabilise them until a fully equipped ambulance arrives.

The pilot project also drew attention for deploying trained female volunteers, a move aimed at expanding community-based emergency care while encouraging greater participation of women in rescue services.

Social media remains divided

While many online users continued to ridicule the initiative, others argued that the criticism overlooked the realities of emergency response in a city where traffic congestion frequently slows ambulances.

Supporters said the bicycles offer a practical, affordable solution for reaching patients in locations where larger emergency vehicles struggle to gain access. Several users also pointed out that similar first-response bicycle units have been used in crowded urban areas and at large public events in different parts of the world.
