Kolkata: The application process for the West Bengal government's upcoming Annapurna Bhandar scheme is set to begin on June 1, with beneficiaries required to furnish extensive details about their family, assets, income and existing welfare benefits through a 13-page application form.

The form, which became available online from the night of May 27 and can also be collected offline from BDO, SDO and District Magistrate offices, seeks information across seven broad categories to determine eligibility for the scheme.

According to the application format released by the state government, applicants must provide details on family identification, food subsidy benefits, household assets, income, identity documents, information about children in the family, and any other government welfare schemes they currently receive.

Chief Minister Suvendu Adhikari said the enrolment process would continue for 90 days from June 1.

He stated that applications would be accepted both online and offline, with municipalities, BDO offices and panchayat-level officials assisting beneficiaries in filling up the forms.

A key section of the application focuses on the household's assets. Applicants will be required to declare whether their home has three or more permanent rooms.

The form also asks whether any member of the family owns land. In cases where land ownership exists, applicants must submit supporting documents, including land registration papers, mutation certificates and the latest land records.

Families will also need to disclose the total landholding of all members and provide updated Record of Rights (RoR) documents.

The form further seeks details on ownership of non-commercial four-wheeler vehicles.

Applicants whose family members own a car must mention the vehicle's registration number and model.

Another disclosure requirement relates to health insurance coverage. Families with health insurance policies will have to provide details of the sum insured and premium amount.

Officials said all sections of the form must be completed and supported with the required documents before submission.

The information collected through the application process is expected to be used to verify eligibility for benefits under the Annapurna Bhandar scheme, which is slated to be rolled out by the state government in the coming months.
