Can ‘nub theory’ actually predict your baby's sex at an ultrasound appointment?

Waiting to find out your baby's sex can feel endless. For most expecting parents, the big reveal doesn't come until the anatomy scan around 20 weeks but a growing number of people are turning to early ultrasound images and a viral concept called "nub theory" hoping to get the answer sooner. Here's what it actually involves, and how much you should trust it.

What is nub theory?

In the earliest weeks of pregnancy, every embryo develops a small structure called a genital tubercle. By the end of the first trimester, this tubercle starts to take shape as the beginnings of either a penis or a clitoris and on an ultrasound, it can sometimes be visible as a small "nub."

Nub theory is the idea that the angle of that nub relative to the fetus's spine can hint at biological sex before it's otherwise visually obvious.

The general rule floating around online: a nub angled more than 30 degrees from the spine is said to suggest a boy, while one closer to parallel with the spine under 30 degrees is said to suggest a girl.

Some proponents believe the nub may be detectable as early as 9 weeks, when the tubercle first begins differentiating, but most agree the theory only becomes meaningfully useful starting around week 12.

So, how accurate is it, really?

It depends on who's looking, and when. A few small studies have tried to measure nub theory's accuracy, with mixed results.

Research from the late 1990s tracking roughly 170 pregnancies found accuracy climbed sharply with gestational age, starting around 70% at 11 weeks and reaching close to 100% by 13 weeks.

A larger study of over 600 pregnancies in the mid-2000s found similar trends, with accuracy around 90% at 12 weeks and nearing 100% by 14 weeks.

But a more recent study complicates that rosy picture. Research from 2012 found that when ultrasounds were read before 11 weeks even by trained professionals, nub theory got it right only about 30% of the time, essentially a coin flip skewed by chance.

Accuracy did improve meaningfully after the 12-week mark, but the takeaway is clear, nub theory is not a reliable early-pregnancy tool, and it's especially shaky in the first trimester.

Factors that can throw off a nub theory reading

  • How the fetus is positioned during the scan
  • The type/quality of ultrasound equipment used
  • The sonographer's experience level
  • How far along the pregnancy is at the time of the scan

For parents who'd rather skip the guesswork, medical experts point to a few options that are considerably more accurate than eyeballing an ultrasound angle:

  • Cell-free DNA (cfDNA) blood test: Also known as noninvasive prenatal testing, this optional blood screening can be done as early as 10 weeks. It's primarily used to screen for chromosomal conditions like Down syndrome, but it reliably reveals fetal sex as a byproduct.
  • CVS or amniocentesis: These diagnostic procedures, chorionic villus sampling (typically done between 10–13 weeks) or amniocentesis (15–20 weeks) are usually reserved for higher-risk pregnancies, but sex can be confirmed from either result.
  • Mid-pregnancy anatomy scan: The standard ultrasound performed between 18–22 weeks remains one of the most dependable ways to determine sex visually, since anatomy is far more developed and easier to interpret by that point.

If certainty matters to you, a blood test or your mid-pregnancy scan will get you a far more trustworthy answer than measuring angles on a grainy 12-week image.
