Is mouthwash bad for your oral health?

Social media is full of alarming claims about mouthwash that daily use wipes out the good bacteria in mouth and raises the risk of diabetes, high blood pressure, heart disease, and even reduced blood flow to the brain. Is there anything to this?

What the evidence shows

Using an over-the-counter mouthwash occasionally say, once a week is unlikely to cause lasting harm. The real concern is heavy, long-term use of swishing certain rinses multiple times a day for months or years may carry some downsides.

Mouthwashes fall into two categories.

  • Therapeutic mouthwashes contain active ingredients that kill bacteria and target problems like bad breath, gum disease, or decay.
  • Cosmetic mouthwashes simply mask odor without any germ-killing effect.

Good oral care means removing harmful bacteria while preserving the beneficial kind work mostly done by brushing, flossing, and regular cleanings. But some mouthwashes, especially prescription rinses containing the antiseptic chlorhexidine, can change the oral microbiome and disrupt its normal functions.

One small trial found that after adults rinsed twice daily with a chlorhexidine mouthwash for a week, their saliva microbiome shifted noticeably, their mouths grew more acidic (raising cavity risk), and bacteria linked to healthy blood pressure declined.

It's less clear whether common over-the-counter ingredients, cetylpyridinium chloride, essential oils, iodine, or hydrogen peroxide disrupt the microbiome to the same degree, though limited evidence suggests prolonged use could have a similar effect. It's wise to avoid continuous use unless treating a specific issue.

There's a plausible mechanism behind the blood-pressure concern- oral bacteria convert dietary nitrates into nitric oxide, which helps relax blood vessels.

Killing off those bacteria could interfere with that process. Some studies link long-term, twice-daily mouthwash use to higher rates of Type 2 diabetes too, though these studies are limited and don't prove cause and effect.

Beyond the microbiome, alcohol- or peroxide-based rinses can irritate the gums, tongue, and inner cheeks, and ingredients like essential oils or foaming agents have occasionally caused burning, ulcers, or tissue peeling.

Choosing a mouthwash wisely

If you need mouthwash for a specific problem, choose a targeted product rather than an all-purpose rinse with multiple active ingredients:

  • Cavity-prone: A fluoride rinse helps and is safer long-term since it strengthens enamel rather than killing bacteria.
  • Dry mouth: Look for a rinse designed to stimulate or mimic saliva.
  • Post-dental-procedure:, unable to brush or floss normally? A short course of therapeutic mouthwash can bridge the gap.
  • Sensitive gums, teeth, or sores: Choose alcohol-free formulas and go easy on whitening rinses.
