A bungee jump that happened without any sign of danger turned into a nightmare in Brazil when a 21-year-old woman plunged more than 130 feet to her death after staff allegedly failed to attach the very rope meant to save her.

The fatal incident has triggered a criminal investigation and sparked fresh concerns over safety standards in adventure tourism activities.

Maria Eduarda Rodrigues de Freitas, 21, died during a bungee jumping session from Skeleton Bridge in Brazil's Sao Paulo state. Disturbing videos from the scene, now circulating widely online, show what appears to be a standard pre-jump procedure.

Staff members can be seen guiding Maria toward the edge of the bridge. There is no visible panic, confusion or indication that anything is wrong. The activity appears to proceed smoothly — until seconds after she is released.

Only then do horrified witnesses realise that the essential safety rope had not been connected.

Emergency responders rushed to the location but were unable to save her. Maria was pronounced dead at the scene, and her body was later transferred to the Legal Medical Institute for examination.

The tragedy has intensified scrutiny because of what the videos seem to suggest: that multiple checks ordinarily associated with high-risk adventure sports may have failed without anyone noticing until it was too late.

Adding to the heartbreak, Maria had posted photographs from the jump site shortly before the incident.

One of the images reportedly showed the bridge along with the wristbands issued for the activity. According to media reports cited by the Daily Mail, she captioned the post: "Who was the crazy person who let me come jump off a bridge?"

The remark, intended as a light-hearted expression of nervous excitement, has since gone viral following news of her death.

Local reports also said Maria's fiance witnessed the incident. He reportedly required medical attention after learning that she had died.

Authorities moved swiftly after the fatal fall. Six people were arrested at the scene as investigators began piecing together exactly how such a lapse could have occurred.
